Services for children and young people with a disability
We have a specialised team of social workers / care managers and local area co-ordinators who work with children and young people, aged 0-18, who have a substantial and permanent disability or medical condition. The teams are based at various offices around Aberdeenshire.
The team works in partnership with other agencies. They help families:
- identify and prioritise their concerns
- learn about service options, and
- understand and plan for their child's needs
This process is called an assessment and may lead to the provision of services if appropriate. If services are provided, they will be monitored and reviewed by the member of the children with disability integrated team assigned to your care.
Services are provided in order to support families and avoid, whenever possible, disruption to family life. Services should seek to minimize the effect of the child's disabilities and help children to lead lives which are as normal as possible.
