Community dementia team

 

Who are we?

Young Man Hugging His GrandmotherWe are the Community Dementia Team, supporting people who have been diagnosed as having dementia, your family and carers. We aim to ensure that you and your carers receive services tailored to meet your needs and that help you maintain your quality of life.

We provide a ‘one-stop shop’:

  • assessing your needs
  • purchasing care packages
  • providing services, and
  • being a focal point for liaison with other service providers on your behalf

We will carry out an assessment of your needs and identify the support you require.

We have access to services from private and voluntary agencies as well as our ‘in-house’ services, such as psychiatric, social work / care management and nursing liaison services.

What we can offer you

  • specialist support workers
  • home care
  • help with personal care
  • small groups / day care
  • social experiences, such as shopping, lunch trip, car outing
  • housing support
  • home based respite
  • help with moving into long term care
  • regular reviews of placements
  • advice and information (benefits, finance, power of attorney, etc)
  • ongoing emotional support and counselling
  • guidance on working with people who are very physically disabled
  • general support to carers
  • a carers support group
  • carer relief

We can also provide access to:

By providing this range of services we aim to prolong the length of time that you can remain living in your own home. When this is no longer possible, we will assist you with moving into long term care and will support you and your carers through the moving process.

What is an assessment of needs?

Two women talkingWe will meet with you and your carer to discuss your circumstances (health, housing, financial) and your needs.  Together, we can decide what support and services you require.  This is an assessment of needs.

Once we have agreed the services that best meet your needs, we can co-ordinate them on your behalf.

We can also carry out a general risk assessment to identify if you may have any problems with kitchen safety, fire hazards, or lifting / moving objects.

We will regularly review the services you receive to ensure your ongoing needs are being met.

Your carer may also benefit from extra help and support. If they want, we can offer them a carer’s assessment.

How can I be referred to you?

You must have been diagnosed by a GP or consultant psychiatrist as having dementia before you can be referred to us.

Our administration office (room 11) is directly ahead of you when you enter the Staff Home. We have a small, quiet room with easy chairs and tea / coffee making facilities, as well as two larger meeting rooms. There is disabled access to the building.

There is plenty of parking, including disabled spaces, in the Hospital’s two car parks. You are not able to park directly outside the Staff Home.

You can be referred to the Inverurie team if you are registered with health practices in Inverurie, Kemnay, Ellon, Oldmeldrum, Fyvie, Tarves and Westhill. You must have been diagnosed as suffering from dementia.

There is no parking. You can park at the public pay and display car park in Nelson Street which is around the corner from the office.

You can be referred to the Huntly team if you are registered with health practices in the Huntly, Alford, Insch, Strathdon and Rhynie areas. You must have been diagnosed as suffering from dementia.
