Services for people requiring palliative care
People living at home or in residential care will be supported by their GP and community nursing service with specialist support available from Marie Curie nurses and Macmillan Cancer Support.
Our social work services work in partnership with health services and the voluntary sector to provide a social work and home care service for people requiring palliative care.
A member of the primary care team will manage your care in collaboration with social work. If it is considered necessary, the primary care provider assigned to your care will make a referal to social work for you. All care provided by social work services is subject to an assessment.
On completion of the assessment, the social work team member assigned to your care will discuss your needs with you and your family. They will provide you with information about mainstream services and specialist services. Once your needs are identified they will agree a care plan and make arrangements to implement services and any other assistance required as part of the care plan. A typical plan may look at aspects of practical, financial and social support.
If you need help to look after yourself, or a relative or friend needs help, you should discuss this with the district nurse who will contact your local social work office.
If you are in hospital or attending a clinic, you can ask nursing staff to speak to social workers based there.
