Consent Forms
Most Aberdeenshire youth programmes require the completion of a consent form by a parent or guardian for young people under the age of 16. Once they reach 16 young people may sign their own form.
Keeping young people safe is a key priority for staff working with young people, which is our main reason for using consent forms. In the case of a medical emergency it is important that we have information on any medical conditions or allergies. We also require permission for the administration of anaesthetic and a contact telephone number so we can notify parents / guardians should an emergency occur.
Photographs or video are often used by our youth groups for publicity and evaluation purposes and we are required by the Data Protection Act to obtain signed permission for these.
